Bhagavad Gita 8.6

Krishna speaking to Arjuna

यं यं वापि स्मरन्भावं त्यजत्यन्ते कलेवरम्। तं तमेवैति कौन्तेय सदा तद्भावभावितः ॥

yaṁ yaṁ vāpi smaran bhāvaṁ tyajatyante kalevaram taṁ tam evaiti kaunteya sadā tad-bhāva-bhāvitaḥ

Whosoever at the end abandoneth the body, thinking upon any being, to that being only he goeth, O Kaunteya, ever to that conformed in nature.

In plain words: Krishna tells Arjuna, called Kaunteya (son of Kunti), that whatever a person thinks about most when they die is what they become after death.
